Your First Medicare Window, Step by Step
Your 65th birthday brings celebrations and, often, a flood of insurance mail from companies that know exactly when it is. Beneath all that sit a handful of true deadlines, and missing one can mean paying more permanently.
We help you lay out the months around your birthday, decide whether Part B should start now and choose the right coverage once you are enrolled.

We do not offer every plan available in your area. Any information we provide is limited to those plans we do offer in your area. Please contact Medicare.gov or 1-800-MEDICARE to get information on all of your options.
The Initial Enrollment Period
This seven-month window surrounds your birthday month, running three months before and three after it. Enroll in its first three months and your Medicare starts the day your birthday month opens.
Anyone already receiving Social Security retirement benefits usually gets Parts A and B without filing anything, and the red, white and blue card arrives in the mailbox. Everyone else applies with Social Security.
Penalties Worth Knowing
- Part B: an extra 10 percent on the premium for every complete 12 months you could have had it and did not, typically permanent
- Part D: a monthly penalty for each month without creditable drug coverage after your window ends
- Medigap: your guaranteed right to buy a policy ends after your six-month window
Still covered through a large employer where you or your spouse works? You may be able to put off Part B and enroll later through a Special Enrollment Period. We look at how your job-based plan works with Medicare first.
Picking Your Coverage
With Parts A and B in place, the next step is filling the gaps. Route one joins supplement coverage with stand-alone Part D; route two is choose a single Medicare Advantage plan. We weigh both against your medicines and doctors.

I'm 64 and working at a hotel on the Boardwalk. Should I enroll now?
It depends on your employer's size and how its health plan coordinates with Medicare. We go over it with you, and Social Security can confirm.
My husband is younger. What happens to his coverage?
Medicare covers individuals, not couples, so he may need to keep job-based or marketplace coverage until he becomes eligible.
Can the entire process be in Spanish?
Yes. We can explain and review every step in Spanish.
